SMILE Quick Facts
What Is SMILE?
SMILE (Small Incision Lenticule Extraction) is the newest generation of laser vision correction, FDA-approved in 2016. Unlike LASIK, which creates a large flap in the cornea, SMILE uses a femtosecond laser to create a small lens-shaped piece of tissue (lenticule) within the cornea, which is then removed through a tiny 2-4mm incision.
Key difference from LASIK: No flap is created. The 80% smaller incision preserves more corneal nerves, potentially resulting in less dry eye and greater long-term corneal stability.
How SMILE Works
- Lenticule creation: A femtosecond laser creates a thin, lens-shaped piece of tissue within the cornea
- Small incision: The same laser creates a tiny 2-4mm incision
- Lenticule removal: The surgeon removes the lenticule through the small opening
- Cornea reshapes: Removing the lenticule changes the cornea's curvature, correcting vision
The entire procedure takes about 10-15 minutes for both eyes.
SMILE vs LASIK: Comparison
| Factor | SMILE | LASIK |
|---|---|---|
| Incision Size | 2-4mm | ~20mm (flap) |
| Flap Created | No | Yes |
| Dry Eye Risk | Lower | Moderate |
| Corneal Stability | Higher | Good |
| Treats Hyperopia | No | Yes |
| Enhancement Ease | More complex | Easier |
| Recovery Speed | 24-48 hours | 24-48 hours |
| Colombia Price | $2,000-$3,500 | $1,049-$1,500 |
Who Is a Good Candidate for SMILE?
SMILE is ideal for:
- Myopia (nearsightedness): Up to -10.00 diopters
- Astigmatism: Up to 5.00 diopters
- Patients prone to dry eye
- Active lifestyles (sports, military) who want maximum corneal stability
- Age 18+ with stable prescription
SMILE is NOT suitable for:
- Farsightedness (hyperopia) — LASIK is the better choice
- Patients who may need future enhancements (LASIK enhancements are easier)
SMILE in Colombia
Top clinic for SMILE: Instituto de Córnea in Bogotá has the ZEISS VisuMax 800 — the most advanced femtosecond laser platform for SMILE procedures. This is the same technology used at the world's leading eye centers.
Pricing: $2,000-$3,500 for both eyes in Colombia vs. $5,000-$8,000 in the United States.
Why the higher price than LASIK? SMILE requires specialized, newer equipment (only Carl Zeiss makes the VisuMax platform) and has higher technology licensing costs.
SMILE Recovery
Recovery is similar to LASIK:
- Day 1: Vision noticeably clearer, though some haziness normal
- Days 2-3: Most patients see well enough to resume normal activities
- Week 1: Vision continues stabilizing
- Months 1-3: Full visual stabilization
Dry eye: Studies show SMILE patients typically experience less dry eye than LASIK patients, and symptoms resolve faster. This is one of SMILE's main advantages.
Should You Choose SMILE or LASIK?
Choose SMILE if:
- You're prone to dry eyes or have borderline dry eye
- You only have myopia and/or astigmatism (not farsighted)
- You want maximum corneal stability for active lifestyle
- You're willing to pay the premium for newest technology
Choose LASIK if:
- You have any degree of farsightedness
- You want the lowest cost option
- You may need enhancement procedures in the future
- You have no significant dry eye concerns
Both procedures have excellent outcomes. The best choice depends on your specific eyes, prescription, and preferences.
