Dry eye is the most commonly reported side effect after LASIK โ and it's also the most predictable, manageable, and, in the vast majority of cases, temporary. Studies consistently show that 40 to 60% of LASIK patients experience some degree of dryness in the weeks to months following surgery. But the numbers need context: most dry eye after LASIK is mild, peaks in the first one to three months, and resolves by six to twelve months in all but a small minority (under 5%) of patients.
Why LASIK Causes Dry Eye
The answer is neuroanatomy. The corneal flap created during LASIK severs superficial corneal nerves that are responsible for the reflex tear production loop. When these nerves are cut, the cornea temporarily loses some of its ability to sense dryness and trigger tear production. As the nerves regenerate over three to six months (and sometimes up to twelve months), normal tear function gradually returns.
This is why procedures that cut fewer nerves โ like SMILE (which uses a small incision instead of a full flap) and ICL (which doesn't touch the cornea at all) โ have significantly lower dry eye rates.
| Procedure | Corneal Nerves Affected | Dry Eye Incidence | Typical Resolution |
|---|---|---|---|
| LASIK | Most (flap severs ~70%) | 40โ60% | 3โ12 months |
| SMILE | Fewer (~20% fewer than LASIK) | 20โ30% | 1โ3 months |
| PRK | Moderate (surface healing) | 30โ40% | 3โ6 months |
| ICL | None (no corneal procedure) | Minimal | N/A |
Managing Post-LASIK Dry Eye
First Line: Preservative-Free Artificial Tears
The cornerstone of management. Use preservative-free formulations (Refresh Optive, Systane Ultra preservative-free, or equivalent Colombian brands) liberally โ every one to two hours in the first month, then as needed. Preserved drops can actually worsen irritation with frequent use due to the preservative chemicals.
Environmental Adjustments
Humidifiers in air-conditioned spaces, the 20-20-20 rule for screen time (every 20 minutes, look at something 20 feet away for 20 seconds), wraparound sunglasses outdoors, and conscious blink reminders during concentrated tasks all reduce evaporative dry eye.
Omega-3 Supplementation
Evidence suggests that oral omega-3 fatty acids (2,000 to 3,000mg EPA/DHA daily) improve tear film quality and reduce inflammation. Some surgeons recommend starting these supplements two to four weeks before LASIK and continuing for three to six months afterward.
Prescription Options for Persistent Cases
If dryness persists beyond three to six months, prescription options include cyclosporine drops (Restasis or generic), lifitegrast (Xiidra), punctal plugs (tiny silicone inserts that block tear drainage, preserving your natural tears on the eye surface), and in rare cases, autologous serum tears made from your own blood.
When to Be Concerned
Mild to moderate dryness in the first three months after LASIK is completely normal and expected. You should follow up with your surgeon or eye doctor if symptoms are getting worse rather than better after three months, if you have significant pain rather than just irritation, if your vision fluctuates substantially throughout the day (indicating severe tear film instability), or if you develop any signs of infection (redness, discharge, light sensitivity).
Concerned About Dry Eye?
Discuss your tear film health with a Colombian refractive specialist before choosing a procedure. A proper evaluation helps match you to the option with the lowest risk for your eyes.
