- Mechanism: The laser breaks melanin particles from the iris surface. These particles float into the aqueous humor and must be cleared by the trabecular meshwork (drainage system).
- The problem: Massive pigment release overwhelms the drainage system, causing pigmentary glaucoma — elevated eye pressure that damages the optic nerve.

Cosmetic Iris Implants (e.g., BrightOcular, NewColorIris)
A flexible silicone disc is surgically placed over the natural iris inside the eye. Originally developed for patients with iris defects (aniridia, coloboma), these devices have been marketed for purely cosmetic color change.
- The problem: The implant rubs against the iris, corneal endothelium, and lens. This continuous contact causes a cascade of damage.
Documented complications:
- Corneal endothelial cell loss (leading to corneal edema requiring transplant)
- Chronic uveitis and secondary glaucoma
- Cataract formation
- Iris atrophy
- Hypopyon (pus in the eye)
- Explantation surgery (removal of the implant, often with permanent damage already done)

Why Some Clinics Still Offer These Procedures
Eye color change procedures continue to be marketed — often in countries where cosmetic device regulation is less stringent. Key warning signs:
- Claims of "new, safer technology": No published peer-reviewed long-term safety data exists for any cosmetic eye color change procedure
- Patient testimonials as evidence: Social media before/after photos don't show the complications that develop months or years later
- Procedures performed outside hospital settings: Legitimate ophthalmologists don't perform unapproved procedures in unaccredited facilities
- Price-only marketing: Emphasis on cost and aesthetics with no discussion of risks or long-term monitoring
The FDA has not approved any device or procedure for cosmetic iris color change. BrightOcular iris implants were the subject of an FDA warning letter. In Colombia, INVIMA (the Colombian FDA) has not approved these procedures either. Reputable Colombian ophthalmologists will refuse to perform them.
If You've Already Had Eye Color Change Surgery
If you've had a cosmetic iris implant or laser depigmentation and are experiencing symptoms, seek care from an ophthalmologist — not the provider who performed the original procedure. You need:
- Complete eye exam including IOP measurement, gonioscopy, and specular microscopy
- Assessment of corneal endothelial cell count (if below critical threshold, transplant may be needed)
- For iris implants: evaluation for explantation (removal), which is itself a complex surgery with risks
- Long-term monitoring for glaucoma, even if the device is removed
Safe Cosmetic Alternatives
If you want to change your apparent eye color without risking your vision:
