That's presbyopia — the gradual loss of near-focus ability as the eye's natural lens stiffens with age. It happens to virtually everyone, regardless of whether you're nearsighted, farsighted, or had perfect distance vision your entire life.

Presbyopia is the main reason many people assume LASIK isn't an option after 40. The concern is valid but misunderstood: standard LASIK corrects distance vision by reshaping the cornea, but it doesn't address the lens-based focusing issue that causes presbyopia. If you correct both eyes for perfect distance vision at age 45, you'll see beautifully across the room — and need reading glasses for your phone.

Monovision LASIK: The Most Common Solution

Monovision is the workhorse approach for LASIK patients over 40. The concept is simple: your dominant eye is corrected for distance vision, and your non-dominant eye is intentionally left slightly nearsighted (typically -1.00 to -1.75 diopters) to handle near tasks. Your brain learns to prioritize the appropriate eye depending on the task, and most patients adapt within two to four weeks.

Monovision isn't new — ophthalmologists have been prescribing monovision contact lenses for decades. LASIK simply makes it permanent. The success rate is high: approximately 85 to 90% of patients who trial monovision with contacts adapt well and report satisfaction with the surgical version.

The critical step: Any reputable surgeon will insist on a monovision contact lens trial before performing monovision LASIK. You wear one contact set for distance and one for near for one to two weeks. If your brain adapts comfortably, you're a strong candidate. If you find the imbalance disorienting — some patients report mild depth perception issues — standard LASIK for distance with reading glasses may be the better choice.

Blended Vision: The Premium Approach

Blended vision (sometimes marketed as "PRESBYOND" by clinics using Zeiss technology) is a refinement of monovision. Instead of creating a sharp divide between distance and near eyes, the laser creates an extended depth of focus in both eyes with overlapping intermediate zones. This reduces the adaptation period and minimizes the depth perception compromise that some monovision patients notice.

Not all clinics offer blended vision — it requires specific laser platforms and surgical expertise. In Colombia, the larger ophthalmology centers in Medellín and Bogotá are equipped for it, typically at a modest premium over standard monovision LASIK.

When LASIK Isn't the Right Choice After 40

If your ophthalmologist detects early cataract formation, LASIK is typically not recommended — you'd be reshaping a cornea that will need to work with an artificial lens implant in the near future anyway. In these cases, refractive lens exchange (RLE) or waiting for cataract surgery with a premium intraocular lens (which can correct both distance and near vision) is usually the better path.

Additionally, patients over 55 should have a thorough retinal examination. Higher myopia combined with aging increases the risk of retinal issues, and these need to be addressed before any elective corneal procedure.

LASIK After 40 in Colombia: Cost

The cost for monovision or blended vision LASIK in Colombia is comparable to standard LASIK — typically $1,100 to $2,000 for both eyes, versus $4,000 to $6,000 or more in the US. The monovision contact lens trial is usually included in the consultation fee. For patients over 40, a more comprehensive pre-operative evaluation is standard, including corneal topography, lens opacity assessment, and retinal imaging.

The additional diagnostic workup adds slightly more time to the pre-op process — plan for a consultation day followed by the procedure the next day, rather than same-day evaluation and treatment that younger patients sometimes receive.
